Thank you for the opportunity to provide your company with a quotation. Khanyisa Verification is a SANAS Accredited B-BBEE verification agency. Khanyisa Verification will access and verify the BEE elements, to provide an independent and impartial opinion on the B-BBEE status of your company.

2. BBBEE Verification Information

2.1 General

2.2 Single Entity Verification

2.3 Group Verification if Applicable

Only to be filled out by companies requiring a Group verification.

2.3.1 Do you require a consolidated verification report & certificate incorporating all group companies? 2.3.2 Kindly provide the number of companies within the group that needs to be verified 2.3.3 Kindly provide the staff compliment within the group to be verified 2.3.4 Kindly provide the turnover figures for the group to be verified 2.3.5 Please provide the industry within which each entity within the group falls PLEASE NOTE: The invoice will be made out to the entity whose details have been completed on "Section 1 Application" of this verification application form. Should you want a separate invoice for each entity to be verified, please complete a separate verification form per entity.

3. Impartiality

3.1 To ensure impartiality, and utmost objectivity in the verification process, below is the list of verification personnel that may be involved with the verification process of your company. If there is a relationship between any of the verification personnel and your company, please provide details of such* Details of Relationship* 3.2 Should the impartiality of this process be at risk as a result of any such relationship, Khanyisa will inform you of this. 3.3 In the event of any possibility of such risk to the impartiality of the process, Khanyisa Verifications elects not to conduct the verification process. 3.4 Verification process might be conducted remotely due to Covid 19:

4. Confidentiality

4.1 All information contained or created during the performance of the verification process will be kept confidential. 4.2 Information about your enterprise from sources other than from you will be treated as confidential. 4.3 All information about your enterprise or individuals within your enterprise will not be disclosed to a third party without your or the individual’s written consent. Where Khanyisa Verifications is required by law to release confidential information to a third party, unless prohibited by law, you and / or the individual concerned will be notified in advance of the information provided. 4.4 Where confidential information must be made available to other bodies (for e.g. to the Dti or the SANAS), prior to making this information available, you will be informed. 4.5 We are entitled to disclose your actual verification result without compromising the confidentiality of the information supporting the verification outcome.

5. Feed and Payment Process

5.1 The fees for conducting verifications are generally assessed with reference to the size of your business. We use annual turnover and the number of employees as the most common indicators when quoting for the use of our services. 5.2 Fees exclude VAT. Expenses such as travel and accommodation when necessary may be charged to deal with any unforeseen complexities. Date*

Verification Process

The verification process will entail:

Engagement Meeting

An Introductory meeting to discuss the approach, timing and process (5 days)

Information Gathering

Reliance will be placed on information prepared by the measured entity, in accordance with a request for specific information from Khanyisa Verification (20 Days)

File Compilation

Client prepare documents and email or Courier the file (5 Days)

Documents Review

Documentation prepared by the measured entity will be reviewed in preparation for onsite verification (4 Days)

Site Visit & Audit

Auditing of documentation and scorecard elements will take place on site and will enable verification methodologies to be carried out in accordance with the verification manual. The scorecard will be validated and prepared. (5 Days)

Verification Review

A final review of the analyst’s recommendations & supporting evidence resulting in a decision to award a given BEE Contributions Status (5 Days)

Issue of BEE Certificate and Scorecard

Deliverables: Verified BEE certificate and Scorecard report (5 Days)

Terms of Payment

It is the policy of Khanyisa Verification to require payment of fees prior to commencement of work. This policy is necessary to:

Ensure that the company remains impartial in its verification decisions and activities,
Prevent the company from being placed under undue pressure at the conclusion of verification as a result of fees being withheld in lieu of certificate amendments.
Ensure client commitment to the verification process from the earliest stages of the verification process, and prevent an unnecessary accumulation of work-in progress which will threaten the company’s ability to maintain the requirements of R47-03.

Acceptance

This quotation is an offer to provide the services defined herein. Acceptance of this quotation is indicated by signing below and constitutes a contract between the parties, subject to Khanyisa Verification standard terms and conditions applicable at signature date, a copy of which is attached hereto, and which standard terms and conditions are therefore incorporated in this agreement by reference. Works will commence upon receipt of payment.
